Regulation must precede insight
Trauma, the Nervous System, and the Body’s Memory
Trauma-informed research offers critical insight into why emotional suppression and chronic stress so often manifest as physical symptoms. Psychiatrist and neuroscientist Bruce Perry’s work, particularly in The Boy Who Was Raised as a Dog, emphasizes that trauma is not solely a psychological experience but a physiological one that reshapes the developing and adult nervous system (Perry & Szalavitz, 2006).
Perry explains that the brain develops sequentially and hierarchically, from the brainstem upward. When individuals experience chronic stress, neglect, or trauma, the lower regions of the brain responsible for survival remain overactivated. In these states, the body prioritizes threat detection over digestion, emotional processing and cognitive integration. This has profound implications for both mental and physical health.
When the nervous system remains locked in survival mode, bodily systems such as digestion, immune function and hormonal regulation are often compromised. This helps explain why individuals with histories of prolonged stress or emotional suppression may experience inflammation, gut dysfunction, or chronic illness without a clear medical explanation. The body is adapting to perceived threat.
Perry’s work reinforces a key trauma-informed principle: symptoms are adaptive responses shaped by the nervous system’s attempt to maintain safety. Emotional suppression, in this context, is a learned survival strategy. Over time, however, these strategies can contribute to dysregulation across multiple systems, including the gut–brain axis.
Rhythm, Regulation, and Expression
One of Perry’s most influential contributions is his emphasis on regulation through patterned, rhythmic experiences. He notes that insight alone is insufficient without restoring nervous system regulation through the body. Practices that support rhythm and regulation such as breathwork, movement, consistent routines and safe relational connection help calm the lower brain, creating conditions where emotional expression and cognitive processing become possible (Perry & Szalavitz, 2006).
This framework aligns closely with emerging research on gut health and inflammation. Chronic nervous system activation alters digestion, gut motility and microbial diversity, reinforcing the connection between emotional stress and physical symptoms. Regulation becomes the bridge between psychological experience and physiological function.
When the nervous system feels safe, the body is better able to digest, repair tissue, regulate inflammation and integrate emotional experience. Expression, whether through language, movement, or relational connection, becomes a natural extension of regulation rather than an overwhelming act.
From Suppression to Integration
Perry’s work challenges the idea that wellbeing can be achieved through insight alone. Understanding trauma intellectually does not automatically regulate the nervous system. Instead, wellbeing emerges through experiences that allow the body to move out of survival states and into states that support connection, digestion and restoration.
This reinforces the idea that inflammation, gut health and mental health are not separate conversations. They are expressions of how the nervous system has learned to respond to the world. When suppression gives way to safe expression, and when regulation precedes insight, the body is no longer required to carry what the mind could not process.
Trauma, Regulation, and Polyvagal Theory
The Polyvagal Theory compliments Perry's work and further clarifies why trauma so often manifests as physical illness. According to this framework, the autonomic nervous system constantly scans for cues of safety or danger. When safety is compromised, the body shifts into defensive states such as fight, flight, or shutdown. These states are not choices, but automatic physiological responses (Porges, 2011).
When the nervous system remains in these states for extended periods, functions that support digestion, immune balance and emotional processing are deprioritized. This helps explain why trauma exposure and chronic stress are strongly linked with gut dysfunction, inflammation, autoimmune symptoms and mood disorders.
Within this model, regulation must precede insight. The nervous system must experience safety before the body can reduce inflammation, restore digestion and integrate emotional experience. Treating symptoms without addressing nervous system state may offer temporary relief, but it rarely creates lasting change.
